Does Your Risk Assessment Make Your JHA Optional? For Some Teams, the Answer Is Quietly Yes.

Ask five people on a typical Malaysian worksite whether a Risk Assessment or a Job Hazard Analysis governs a specific task-level decision, and there’s a reasonable chance you’ll get more than one answer. That inconsistency isn’t usually a knowledge failure in the sense of nobody having heard of either document. It’s a hierarchy failure — a quiet, widespread confusion about which document is supposed to do what, and what happens when one is treated as making the other unnecessary.

Where the Confusion Took Root

In this investigation, some members of the team involved treated the Risk Assessment as effectively superseding the Job Hazard Analysis — operating on the belief that because the high-level document had already covered the work programme’s hazards, the task-level document didn’t need to add much beyond a formality. That belief doesn’t hold up against what each document is actually designed to do. A high-level Risk Assessment was never going to specify whether ground-level access was sufficient for one particular task, or whether a specific technician’s physical reach matched what a specific job required. That’s precisely the level of detail a Job Hazard Analysis exists to provide — and if the team believes it’s already been covered elsewhere, that detail simply never gets written.

This is very likely a meaningful part of why the site’s Job Hazard Analysis for this task read as generically as it did. When a team can’t clearly articulate which document governs a specific task-level decision, that ambiguity is exactly the gap where a control measure stays vague and nobody catches it, because everyone assumes the necessary specificity exists somewhere else in the paperwork.

Why This Isn’t Simply a Documentation Preference

It would be easy to treat this as a matter of internal convention — some sites emphasise the Risk Assessment, others lean on the Job Hazard Analysis, and either approach works as long as someone is thinking about the hazards. In practice, the hierarchy matters because the two documents are built at genuinely different resolutions, produced through different processes, often by different people, at different points in the planning timeline. Treating either one as a substitute for the other doesn’t just create redundancy — it creates a specific blind spot at whichever level of detail the “superseded” document was supposed to cover.

The Practical Test

The clearest way to check whether this ambiguity exists at your own site is the question this article opened with: ask five people, across different levels — a supervisor, two technicians, an HSE officer, a manager — which document governs a specific task-level access or method decision on a job they’ve all worked on together. What Closing the Gap Actually Requires

The fix isn’t producing more documents.
