A permit-to-work is one of the most widely used safety controls in Malaysian process facilities. It is also one of the most commonly misunderstood — not in terms of what it requires, but in terms of what it assumes. The assumption that causes the most incidents is this: that the conditions verified at the start of the work will still be accurate when the work continues on a different shift.
The Problem With a Valid Permit Across a Shift Change
A permit is issued at the start of a job. The authorised person walks the area, verifies conditions, signs off, and the work proceeds. In facilities that operate across multiple shifts, that job may still be active when the shift changes and a second crew takes over. The permit is still valid. The authorised person who signed it has gone home.
The second crew inherits a document that says the conditions were safe — at the time the first crew saw them. What they cannot inherit is the direct observation and verification that the original authorised person conducted before they signed. The permit is live. The verification behind it is not.
This gap is not theoretical. It is a routine feature of how many PTW systems operate in Malaysian petrochemical, oil and gas, and manufacturing facilities — because the PTW procedure was designed around jobs that start and finish within a single shift. For jobs that don’t, the system has no instruction for what to do when the crew changes.

In Tripod Beta’s causation chain, incidents that emerge from this gap typically trace through a recognisable sequence. The Substandard Act is the most visible element: a second-shift worker proceeds with a task without re-verifying the permit conditions. Investigations that stop here identify the act and recommend retraining or reminders about PTW compliance.

For hot work, confined space entry, and electrical isolation in Malaysian process facilities, DOSH regulations and industry standards place accountability for PTW conditions on the authorised person. When a job runs across a shift change, there is a regulatory gap: the authorised person who verified the conditions is no longer on site, but the work continues under their authorisation.
Most PTW procedures in Malaysia do not have a defined handover protocol for this scenario. Some facilities handle it informally — the departing technician briefs the incoming one verbally. Others have no standard at all. Without a defined, documented handover that transfers responsibility and re-verifies conditions, the gap between what was authorised and what the incoming crew actually knows is invisible to the safety management system until something goes wrong.
Common Investigation Errors This Produces
When an incident occurs during the second shift of a multi-shift job, the investigation frequently locates the cause at the individual level: the operator who made an error, the supervisor who failed to brief the team. Corrective actions target those individuals.
What the investigation missed is that the next multi-shift job, with a different crew, will encounter the same absent handover procedure. The Procedures BRF has not been addressed. The conditions for the same Precondition to develop again are unchanged.
The Structural Fix for PTW Shift Handover in Malaysia
A Procedures BRF is corrected at the procedure level, not at the individual level. The fix for a PTW shift handover gap is a defined handover protocol — built into the PTW document itself — that specifies what must happen before the second shift continues the work. This typically includes a re-verification step in which the incoming authorised person physically walks the conditions, a transfer of authorisation recorded on the permit, and a requirement that no work continues until this step is completed.
Small in scope, significant in effect. The PTW now reflects what actually needs to happen, rather than assuming the conditions it documented at 7am still apply at 3pm with a different team in the area.
The Question for Your PTW Process
If you have a job running across a shift change in your facility right now, ask: what procedure tells the incoming crew what to verify before they continue? If the answer is “the supervisor briefs them” — you have a human-dependent handover, not a procedural one. That distinction matters when the supervisor is busy, absent, or when the briefing is compressed by shift-end pressure.
